During the audit of Line A on 2024-07-15, torque wrench #TW-05 (Serial: XZ789) used for critical fastening operation (Part No: ABC-123, Step 5) was found to have a calibration due date of 2023-10-15. The last calibration was recorded on 2022-10-15. This does not comply with internal procedure CP-CAL-002, requiring annual calibration for such equipment.

Torque wrench #TW-05 was immediately removed from service and tagged as 'DO NOT USE' on 2024-07-15. All products assembled using this wrench on 2024-07-15 (Batch #P-20240715-A) were quarantined pending verification.
Corrective Action & Evidence:
Torque wrench #TW-05 was sent for external calibration on 2024-07-16. Calibration certificate #CAL-TW05-20240718 received. Maintenance procedure CP-CAL-002 updated to include weekly check of calibration due dates for critical equipment.
Root Cause Analysis & Evidence:
Root cause identified as oversight in manual tracking system for calibration dates. Contributing factor: Lack of secondary reminder system. (Ref: RCA Report RCA-2024-015)
Preventative Action & Evidence:
Implemented a digital calibration management system with automated reminders (System: CalMaster Pro). Conducted refresher training for all maintenance staff on calibration procedures and importance on 2024-07-20.
